When life gets complicated, it’s our goal to surround you and your loved ones. Your dedicated care team coordinates a community of services to support your unique condition.
By developing trusting, respectful relationships with our patients, their caregivers, loved ones, and other physicians, we can ensure that everyone is committed to supporting the patient’s goals. This approach gives the people in our care the feeling of being listened to, loved, and in control of their story.
Proper care takes time, a team, and trust
Your Prospero Care Team
An extension of your primary care physician and/or specialist. They work together with your doctor(s) to ensure you receive proper care and address any serious health issues that may occur.
Your nurse practitioner or physician assistant is the first member of the team you meet. They prioritize assessing your health, addressing prescription needs and discussing any health and safety concerns.
You can call our care center any time, day or night. We are here to address any questions you have and provide you with the kind of care you deserve.
Your registered nurse meets you in your home for scheduled visits or whenever you need care. They help address medical needs, answer questions, and help communicate concerns to your doctors.
Care Support Specialist
Your care support specialists coordinate your Prospero team’s efforts, making sure your care and activities are organized and your concerns are addressed when they arise — 24 hours a day, 7 days a week. They are always available.
Your social worker is there to provide extra resources and support when you need it. They are an integral part of the care team that helps guide you in the next chapters of your life so you feel fulfilled and connected to your community.
