Your Prospero care plan is created with family at its center. We support our patients and their family members with physician-led care teams that treat the whole person face-to-face or with virtual telemedicine care, while remaining available 24 hours a day, 7 days a week.
This alleviates the burdens that tend to fall on loved ones in times of uncertainty. That means family has the time to just be family, knowing their unique needs are cared for by a trusted community of medical experts.

Prospero Physician
Your Prospero Doctor specializes in caring for people with complex and chronic serious illness. They ensure you receive proper care and address any serious health issues that may occur.
Nurse Practitioner
Your Nurse Practitioner or Physician Assistant is the first person on your care team that you will meet. They will assess your health, address any prescription needs, and support your ability to remain at home.
24/7 Support
You can call our care center any time, day or night. We are here to address any questions you have and provide you with the kind of care you deserve.
Registered Nurse
Your registered nurse meets you in your home for scheduled visits or whenever you need care. They help address medical needs, answer questions, and help communicate concerns to your doctors.
Care Support Specialist
Your care support specialists coordinate your Prospero team’s efforts, making sure your care and activities are organized and your concerns are addressed when they arise — 24 hours a day, 7 days a week. They are always available.
Social Worker
Your social worker is there to provide extra resources and support when you need it. They are an integral part of the care team that helps guide you in the next chapters of your life so you feel fulfilled and connected to your community.
