Required Qualifications

  • Current, unrestricted NP license (or the ability to obtain a Physician’s Assistant license) in the state of assignment
  • Ability to obtain DEA licensure / Prescriptive Authority post-hire in states where applicable
  • Nurse Practitioner national certification as ANP, FNP, or GNP

Primary Responsibilities

  • Provide quality care for patients in their homes in order to manage severe and advanced illness
  • Collaborate with clinical and interdisciplinary care teams to create a comprehensive care plan that will allow individuals living with serious illness to feel empowered and be able to live their best lives now
  • Contribute to a community where care is determined by patient choice and preference, not dictated by circumstance
