Prospero Physician

Your Prospero Doctor specializes in caring for people with complex and chronic serious illness. They ensure you receive proper care and address any serious health issues that may occur.

Nurse Practitioner/ Physician Assistant

Your Nurse Practitioner or Physician Assistant is the first person on your care team that you will meet. They will assess your health, address any prescription needs, and support your ability to remain at home. 

Registered Nurse

Your Registered Nurse meets you in your home for scheduled visits or whenever you need care. They help address medical needs, answer questions, and help communicate concerns to your doctors. 

Social Worker

Your Social Worker is there to provide extra resources and support when you need it. They are an integral part of the care team that helps guide you in the next chapters of your life so you may feel fulfilled and connected to your community.

Care Support Specialists

Your Care Support Specialists coordinate your Prospero team’s efforts, making sure your care and activities are organized and your concerns are addressed when they arise — 24 hours a day, 7 days a week. They are always available to take your calls and assist you in your needs.
